i thought it to be a corer...but it has a spike sticking up in the middle.
Its a corer of some type- You stick it in, twist a little & the "tit" holds whatever it is to allow retreival-
A tamper proof screw driver?
if it didn't have the small spike sticking into it, i might have thought an apple corer. i have seen one before, but can't remember where.
Isn't that an old school tool for cutting plugs from wood similar to this?
too small to be a cookie cutter.
not sharp enough to be a leather punch.

very similar, but i believe a bulb auger would be much larger in the coring diameter.
Hand tool for sure, wood working blade edge, 3 1/2 inches long + the handle, not sure what the punch is in the center for (most likely to keep it straight in a drilled hole), I would say it was for cutting holes in wooden doors for the door handle to go through.
well, i did what most would do. i got an apple and tried corering it.
it cored it.
its an apple corer. the spike acts as a blade and helps pull out the guts when you go all the way thru. it sorta churns the core to make it soft.
so there ya go...apple corer.
i looked inside the blade. pat date...4/1910.
