It's important to know your specific legal circumstances, like for instance, I would normally have to answer (yes) on Question, 13 or 14 (can't recall which) but in Texas, I completed a Texas only prerequisite that legally means I was only charged but never convicted, so because I was charged but not convicted means legally I answer that specific question (no). So even after reading the front of the document, you may need to read the index on the back, because it's really not as black and white as it may first appear.

Disclaimer- I am NOT a lawyer (am skilled labor)

This is a link to the form 4473. The instructions are on the back pages.

http://www.atf.gov/forms/download/atf-f-4473-1.pdf

Regarding the question on mental illness-

ADJUDICATED means that a judge, board, commission, etc found that you were a danger to self or others, or were incompetent or unable to manage own affairs. It includes being committed to a mental institution by order of a judge, commission, board, etc. It does NOT include someone that sought out medical care, counseling etc.

For awareness, the version of ATF form 4473 mentioned in this thread has recently been superceded and is therefore obsolete. The latest version is making its way to an FFL near you at this time.

01, you are correct- did not have the newest one yet- thank you.

The Section 11f (mental incapacity) is not changed. What DID change was Resident Aliens no longer have to prove they have resided here 90 days.

Section 10 has been broken down into 10A and 10B. Verbage for 11L has been changed (this was the main reason for the update). Due to 11L changing 12 needed to change as well.
