That sounds about right (though I prefer more than 6" of distance), I would not change the spring or adjust extractor.
We eliminated the magazine issue.
We know it is not a lube issue.
We know it is not a cleaning issue.

Ammo? That would be a slim chance, but still a possibility.
Weak hold? Could be an issue.

I would take pictures of the next malfunction. Have someone take of picture of your hold, also. Then coordinate with Para-USA.

thanks for the advice this is my first full size handgun and 1911. i fixed my accuracy issues but i could have a weak hold on it. ill test out alot of stuff at the range and see what happens. thanks guys. if i have another one ill take a picture of it with my hold and post it.

Most of the time it is the magazine that causes it i use mc cormick mags they really work good

Some of the issue has been the ammo i was using after switching to Fiocchi my gun handles them flawlessly. Im having the feed ramp polished as i type this and will hopefully have the gun back today. Also having some trigger work as there is alot of side to side movement in my trigger but i may have to buy a new trigger... anyway thats the update for now.

Got the gun back and the buddy that was doing the polishing has shown me an issue the requires more... skilled hands to fix. Where the feed ramp and barrel meet there is a small lip about 1mm of extra metal. He suggest biting the bullet and going to a gunsmith to have that fixed. ill probably go to one and see what they recommend as far as my trigger its not as adjustable as i was hoping so i will be needing a new trigger. If Para cared about their customers and actually answered emails, their phone, or messages i might have answers to my many questions sooner but it seems they dont give a ****. I will be doing an official review of Para Usa's customer service ability in the near future when they actually will talk to me. Anyway thats the update as of today.

Don't remove that.
The barrel should be a little short of the feed ramp. Approx 3/10".
Take it out and try it.

There is supposed to be a small metal shelf below the leading edge of the barrel throat.
