Not real sure on how to answer this as it's almost ridiculous to think you would put in earplugs in a firefight (especially Infantry). I would guess that maybe some would try to use earplugs but I personally didn't know any.
There's too much going on and you really need to be able to hear everything around you.
Some deafness is going to occur if you spend much time around weapons firing and some are luckier than others on the amount of hearing loss but you can almost be assured there is some loss.
I feel lucky to only have some high end loss and part of that could be from being a 96 decibel freak with my music.
Walking into a battalion sized box ambush of NVA in a 113 man company causes semi deafness and loss of bladder control as a side symptom.
The little narrow side streets in Mogadishu would echo shooting so bad you would be partially deaf (and ears ringing for hours).
To answer your question, you deal with the noise.
