Default Renewal ?

On the preprinted C+R renewal form it asks for business phone and fax # and an email address. Are those required to be filled in since we are collectors only?

No. In some cases, such as a museum that holds a C&R FFL, they may have that. Most of us don't.

I'm not that smart and try NOT to assume anything, so I ask dumb questions to get clear answers. Thank you for clarifying it. As C3 says, if you have the C&R as a business yes fill it in, if not N/A the space, for not applicable.
