You should have a copy of the form with your paperwork that you can copy off and put in a binder.

Maybe your ATF Federal Firearms Regulations Reference Guide is newer than mine They sent me a 2005 edition
My Open letter to all recently federally licensed collectors of curio or relic firearms (also dated Jan 19 2005)
on the back page states
Licensed Collectors MUST
Prepare an Acquisition and Disposition (A/D) record as prescribed by
27 CFR 478.125 (f)
section (f) is on page 67
and states nothing of "or an orderly arrangement of loose leaf pages)
also no form to copy and put in a binder was sent in my packet
The notice of Errata for ATF P 5300.4
(updates to the reg book)
say nothing about 27 CFR 478.125 (f) pg 67
I did recive a letter in my packet saying
To Whom it may concern
The ammended FFL form F5300.38was not included in your pack due to it being on backorder. Please call the ATF Distrubution Center on a later date to have one sent to you. Sorry for the inconveinence that this has caused.
