throat problem?

I was out to the range today sighting in some rifles including a remington 223 bolt action. While policing up my brass I came across these two. It made my heart jump until I realized they were not mine (different head stamp). Now I have to wonder who is shooting a rifle with this sort of issue going on in the throat or chamber? Any ideas what might cause this?

Looks like reloads notice the different shoulder lengths. I would say the sizing die was too far down setting the shoulder back leaving enough gap to give a bubble.

Well we know why he didn't police his brass. http://i695.photobucket.com/albums/v...ons/oops-1.jpg
