I bet you are a right strong side. Correct?
My guess....flinch.
Load one cylinder with a snap-cap or just empty. Spin the wheel (Deer Hunter style

) and shoot, with concentration on the front sight
including follow-through.
On the benign trigger pull did the sight picture change?
Your round goes where the muzzle is at the drop of the hammer.
Does the trigger have excessive take up?
My G22 has so much that I acquire the target three times before the damn thing goes off!
