cartridge designation?

What's the second number in the cartridge designation. Like 9x19 or 7.62x39 etc. Is the second number the bullet length or cartridge length?

Bear in mind that the diameter is the NOMINAL diameter. A .22 Magnum is NOT the same diameter as a 22 LR. The 9mm Parabellum and 9mm Makarov or NOT same diameter. Sometimes the name itself is a bit screwy- .38 Specials are NOT .38 inches in diamter, etc.
