Is it safe to fire 9x18 in a semi auto chambered for 9x19

Not a good idea. Probably wouldnt blow up. But may break the extractor. The edge of the case is what the gun headspaces off of. If the case is too short the extractor would be left to hold the case back.

I believe 9x18 is just a bit wider than 9x19 and will not fit.

First, the 9x18 is the 9mm Makarov. Despite the name it is NOT 9mm. It is a 9.2 mm bullet in a 9.91 case.
9x19 is 9mm Parabellum- it is a 9.01mm bullet in a 9.65mm case.
It SHOULD be too fat to chamber, and too short to headspace properly. The energy is lower that a parabellum, made for use in a blowback pistol, not a locked breech pistol. If it DID chamber and DID fire, would likely not cycle action.
Really bad idea.

Makarov is a .363" dia bullet.
Luger/para is a .355-356" bullet.

It won't fit. Had to check it out for myself, so I took the barrel out if my 1911 9mm (STI) and attempted to insert a 9X18. The Round head went in the chamber but stopped at the casing.
Now the 9X19 did go about 3/4 of the bullet length in the chamber of my Makarov.
My conclusion: don't even try to do it.
Safety note: Both guns were field stripped before doing this check.

Also a 9x19 NATO round is tapered and the 9x18 Makarov round is straight walled.
Others already pointed out that the Russian projectile is a larger diameter and would probably become a bore obstruction.
